What entities are eligible to participate in the FSPHP-ETA™ Program?
- Geographic and Operational Considerations
- The FSPHP-ETA™ program is geographically global in scope. Evaluation and/or treatment providers anywhere in the world are eligible to apply for the FSPHP-ETA™ program. An entity must have the ability to tailor and nuance the safety-sensitive concerns unique to practice within the jurisdiction(s) applicable to the entity.
- Entity must be operational for one (1) year under the current policies and procedures or have a minimum of ten (10) participant charts available for review.
- All policies and procedures should be relevant and consistent with the entity’s practice. Any template or “boilerplate” policies used in the production of the entity’s policies should be carefully tailored to describe the actual practice of the entity. If the entity is using rules and/or regulations as its policy, the sections pertaining to the entity must be clearly marked (highlighted).
- At the time of the on-site assessment, consultants used to prepare the facility for the assessment are able to observe the assessment, but should not take part in the assessment.
- Type of Client Served
- The FSPHP-ETA™ program is exclusively for entities that provide evaluation and/or treatment services to healthcare workers in safety-sensitive occupational roles.
- During the application process, entities shall specify the cohort(s) with which the entity claims to have specialized qualification to treat; criteria shall be assessed within the context of the claimed cohort(s).
- Scope of Services Offered
- The FSPHP-ETA™ program is exclusively for entities that either deliver treatment or perform fitness-for-duty assessments/evaluations.
- FSPHP-ETA™ accreditation includes a base credential for both evaluation and treatment entities, with an endorsement module for entities that provide fitness-for-duty evaluations/assessments, and a separate endorsement module for entities that provide treatment. It is mandatory to apply for at least one of these two endorsement modules in addition to the base accreditation. The base accreditation is required for either endorsement module.
- Entities that provide both fitness-for-duty evaluations and treatment may apply for both endorsement modules.
- Entities that perform competency evaluations (of participants’ occupational knowledge, skills, and abilities i.e., fitness-to-practice) but do not provide either fitness-for-duty evaluations or treatment are NOT eligible for FSPHP-ETA™ accreditation.
- Levels of Care Provided
- Entities that provide treatment at an outpatient, intensive outpatient, partial hospitalization, residential, or inpatient level of care are eligible for FSPHP-ETA™ accreditation with the Treatment endorsement in the FSPHP-ETA™ program.
- During the application process, entities applying for the Treatment endorsement shall specify which level(s) of care they have the capacity to provide. That capability will be assessed, and if the entity becomes accredited, the FSPHP-ETA™ accreditation Treatment endorsement shall specify the scope of level(s) of care for which the entity has been accredited.
- Types of Disorders Evaluated/Treated
- For entities applying for FSPHP-ETA™ accreditation with the Evaluation endorsement, the entity must identify the conditions that it evaluates, and it must have the capacity and resources to assess the conditions it has identified - conditions that may lead to impairment, such as: psychiatric disorders, medical disorders and conditions, process disorders, disruptive behaviors, professional sexual misconduct and boundary issues, chronic pain, substance use disorders, and neurocognitive disorders.
During the application process, entities applying for the Evaluation endorsement shall specify what they have the capacity and resources to assess. That capability will be assessed, and if the entity becomes accredited, the FSPHP-ETA™ accreditation Evaluation endorsement shall specify the scope for which the entity has been accredited.
- For entities applying for FSPHP-ETA™ accreditation with the Treatment endorsement, the entity must identify the conditions it treats and must have the capacity and resources to treat the conditions it has identified - conditions that may lead to impairment, such as: psychiatric disorders, medical disorders, process disorders, disruptive behavior, professional sexual misconduct and boundary issues, chronic pain, substance use disorders, and neurocognitive disorders.

- Treatment Modalities
- For entities applying for FSPHP-ETA™ accreditation with the Treatment endorsement, the entity must offer multiple evidence-based treatment modalities appropriate to healthcare workers in safety-sensitive occupational roles and individualized to the participant’s needs.

What are the steps?
- Prepare your Evaluation and Treatment Center:

Purchase FSPHP-ETA™Standards version 2.0
- Review your policy and procedures manuals.
- Perform an internal gap analysis to align the entity’s policies with FSPHP-ETA™ Standards Version 2.0.
- Perform an internal records review to verify that participant charts reflect adherence to your policies and the FSPHP-ETA™ Standards Version 2.0.
- Prepare your FSPHP-ETA™ Application:
The application is the largest lift as it prompts you to review and upload many of your standards, but we promise it is all worth it as the final destination has you ready for your onsite review, and near the finish line of the process!
- Entity applies by filling in an application available on FSPHP website
- Review the template available on the link above to help you prepare for the online application.
- Create an inventory or folder with all the policies & documents that will need uploading to the application.
- Start your application online – IT WILL SAVE AS YOU GO – and once completed, you hit “submit”.
- Now that the application is submitted FSPHP will:
- Randomly assign FSPHP-ETA™ SME Assessor
- Request a Conflict of Interest (COI) w/ SME and Non-Disclosure Agreements
- Send a Doodle poll to schedule the on-site assessment
- FSPHP-ETA™ SME Assessor performs the assessment
- FSPHP provides a report of findings and a corrective action plan if necessary.
How long does the FSPHP-ETA™ Assessment process take?
The FSPHP-ETA™ assessment process typically takes 6 – 8 weeks from application submission to completion. While we strive to maintain this timeline, factors such as program readiness, scheduling conflicts, or unforeseen circumstances may cause minor delays. Our team is committed to supporting you throughout the process to ensure a smooth and efficient experience.

What are the potential results?
- Accreditation Standards
- There are two possible FSPHP-ETA™ accreditation (and reaccreditation) decision statuses/outcomes: FSPHP-ETA™ Accredited or Not Accredited
- Entities that demonstrate substantial compliance with each (i.e., all/every one) of the core/mandatory criteria and at least 50% of the supplemental criteria) shall be FSPHP-ETA™ Accredited and will have a 90-day period to bring all noncompliant supplemental standards into compliance.
- Entities applying for FSPHP-ETA™ accreditation that do NOT demonstrate substantial compliance (as determined by FSPHP-ETA™) with any of the core/mandatory criteria and/or with at least 50% of the supplemental criteria shall be Not Accredited.
