CONTINUING EDUCATION CREDIT

1. Check your email for the conference evaluation link from FSPHP via SurveyMonkey.
All attendees and exhibitors should have received an email with a link to the Conference Evaluation. Please complete and submit this evaluation as your first step. Be sure to check your spam or junk folder if you don’t see it.

2. After submitting the evaluation, check your inbox for a follow-up email.
Once your evaluation is submitted, you will receive a second email containing a link to the Continuing Education (CME) Survey. Completing this brief survey will automatically generate and send your CME certificate.

Please note: there may be a short delay—up to one hour—between submitting the evaluation and receiving the CME survey email. If you still haven’t received it after that time, please reach out [email protected].
